Health Care Program (HCP) for Children with Special Needs
The Children and Youth with Special Health Care Needs (HCP) program is for children and youth with special health care needs and is offered through the Colorado Department of Public Health and Environment. HCP is staffed by nurse-led teams who provide care coordination for children and youth from birth to age 21 with special health care needs. Children can benefit from this program whether their needs are visible, such as a physical need, or invisible, such as a developmental, emotional or behavioral need. Care coordinators connect families with local information and resources that will support understanding and management of their child’s health care.
